Yes, please bring your passport and a passport photograph. If you have your paperwork, please bring it, but if not we can provide it.
It is unlikely that you will fail your visa medical unless there is something significantly wrong with your health. In this case, your priority is to get treatment in the safest possible environment, which is usually your home country.
Yes. If your company is covering the medical, we will need an email or fax of guarantee with full contact details for billing. Alternatively, your company can pay by credit card over the phone prior to your appointment.
