What Are Y Chromosome Deletions?

The Y chromosome carries the genetic blueprint essential for male development and sperm production. Specific regions on this chromosome, known as the AZF regions (AZFa, AZFb, AZFc), contain the “master instructions” for creating sperm. A Y chromosome microdeletion occurs when small regions of the Y chromosome are missing.

This missing genetic material can disrupt sperm production, leading to various fertility challenges. The severity depends entirely on which region is affected:

  • AZFc deletions are the most common and may still allow for some sperm production.
  • AZFa or AZFb deletions are more severe and often result in a complete absence of sperm in the ejaculate (azoospermia)

How Does It Affect Fertility?

For men with Y chromosome deletions, the impact on fertility can range from reduced sperm quality and motility to a low sperm count (oligospermia) or no sperm at all (azoospermia). This testing helps determine not only whether there is a genetic problem, but also how that problem will affect treatment.

Crucially, it helps doctors predict the likelihood of finding sperm directly in the testicles, which is a critical factor for procedures like ICSI (Intracytoplasmic Sperm Injection) during IVF.

What Causes These Deletions?

It is important to understand that Y chromosome deletions are rarely inherited from the father.

In most cases, these are de novo (new) genetic changes. They typically occur spontaneously during the father’s sperm formation or in the early stages of embryo development.

While the exact trigger is often unknown, they can result from random errors during cell division. Because of this, having no family history of infertility does not rule out a genetic cause.

When Is Testing Recommended?

Y chromosome microdeletions are a significant cause of male infertility, affecting approximately 1 in 4,000 men and present in about 5% of UK men with severe sperm production issues. We typically recommend this test if you are:

  • Undergoing investigation for male infertility
  • Diagnosed with a very low sperm count (severe oligospermia) or no sperm in your ejaculate (azoospermia)
  • Preparing for assisted reproductive techniques like IVF or ICSI
  • Considering a family history of male infertility, though most deletions occur spontaneously.

The procedure is quick and straightforward. An experienced doctor or nurse will draw a small blood sample from a vein in your arm, a process that typically takes less than 10 minutes. This sample is then securely sent to our specialised genetics laboratory for analysis using PCR (Polymerase Chain Reaction), which can accurately detect missing sections of the Y chromosome.
